INSURANCE

We try to simplify the preparation of insurance claims for you, thereby holding down costs that are unrelated to good medical care.

We recognize the need for a clear understanding between the patient and his/her doctor regarding financial arrangements for medical and surgical care. The responsibility for payment of these fees belongs to you. We make every effort to keep down the cost of your medical care. We request that you be prepared to pay your medical charge at the time of your visit. Checks may be made payable to Commonwealth ENT We also accept Visa and MasterCard.

We will promptly file your claim with your insurance carrier and direct any payment to you.

Alternative payment arrangements may be made in advance with our financial counselor.

We will automatically file your claims with your insurance company for you. Please provide us with a copy of your insurance card and coverage information.

Payment for services rendered is expected at the time of service. We file primary and secondary insurances as a courtesy to our patients. Co-payments and deductibles are due at the time of service for those carriers that we participate with.
